Job Description:

This position will provide assistance to county referred homeless/disabled individuals/families in the form of case management and linkages with available community resources for furnishing, moving costs, security deposits, application fees, etc.  Will assist participants with lease arrangements and requirements for lease compliance; perform general unit inspections and develop a service plan based on an initial and periodic needs assessment.  Liaisons with local landlords and County government administrators.  Serves as team member of the Permanent Supportive Housing Programs and works cooperatively in meeting unit goals. Employees are at times exposed to abusive, aggressive and unpredictable behavior for a significant amount of time. Maintains timely and accurate client files, inputs information in HOC and County databases, and prepares monthly reports on activities conducted and participant status. This position is a full-time one-year term, grant-funded position; continuation of employment is solely dependent on the approval of grant funding.

Preferred Skills:

• Minimum of Bachelor’s degree from an accredited college or university in Social Work, Human Services or related field.  
• Minimum three years of increasingly responsible experience with Low-income Families
• Ability to relate well to people with diverse backgrounds and cultures
• Must possess good judgment; good computer skills, have the ability to work independently.
• Must possess excellent oral and written communication skill
• Must have good computer skills (Word & Excel).
• Must have working knowledge of community resources.
• Career counseling experience desired. 
• Must have own transportation and good driving record.
